When submitting a complaint regarding the
terms of the Cain v. MDOC Settlements it is
helpful to us when prisoners use the following form to
explain their situation or problem. Print the form
and instructions below and send them to a prisoner you
know that may have issues with classification, access to
courts, personal property, or typewriters.

Note: Please do not write outside of the
boxes and please be as brief as possible when describing
your issue.
Attach photocopies of any documents necessary to
validate your claim, such as greivances, kites, warden’s
forums minutes, misconduct reports, or N.O.I.’s. If you
cannot get copies made, send your originals. We will
return documents if you indicate in #9 that you wish to
have them back.
1. Compliance Code: Write in the Cain monitor case code
that best describes the incident which are you writing
to PLSM about. If you cannot decide which compliance
code to use, or do not have the codes, leave this field
blank.
2. Institution(s): List the institution or institutions
where the incident took place.
3. Prisoner Number(s) or Name(s): List the numbers of
any prisoners who may have witnessed the incident you
are describing. If you do not know their number, please
list their name.
4. Incident Date: The date on which the incident in
question took place.
5. Name(s) of Staff: Include the names and/or titles of
the MDOC staff violating provisions of the Cain
settlement in the incident you are writing about.
6. Incident Description: Describe the violation of the
Cain Settlement which you are complaining about. Be as
clear and brief as possible.
7. Violation: Explain why you think what the MDOC staff
have done is a violation of the Cain settlement.
8. Relevant document(s) enclosed: List the documents you
are including in your complaint as evidence by their
titles. If you are including several grievances, list
their numbers.
9. Return Documents (checkbox): Check the box that
indicates whether you want your documents returned to
you or whether PLSM can keep them.
